The GP Referral programmes:
• Pulmonary Rehab
• Active for Life
• ToneZone Pathway.
These classes link through the Pulmonary Rehabilitation unit and Cardiac Rehabilitation units in local hospitals and community care facilities, mainly in Connolly Hospital. Local GPs can also refer if deemed necessary.
For Cardiac, the participants undergo exercise rehabilitation with their assigned cardiac physiotherapy as part of their initial rehabilitation and following this, they are referred to us if they want to continue exercising.
